Gangwon-do and Six General Hospitals Sign Agreement
"One-stop Support from Discharge to Care"
Strengthening "Gangwon-style Integrated Care"
with 18 Cities and Counties to Help Discharged Patients Return Home
Gangwon Special Self-Governing Province is strengthening its "Gangwon-style Integrated Care" system in cooperation with cities, counties, and major medical institutions in the province, so that patients discharged from hospitals can settle stably in their homes and local communities.
On March 3, the province announced that it had signed a work agreement with 18 cities and counties and 6 general hospitals in the province to implement the integrated care linkage project for discharged patients.
The hospitals participating in this agreement (listed in alphabetical order) are: Gangneung Korea Hospital, Gangneung Dongin Hospital, Taebaek Hospital of Korea Workers’ Compensation & Welfare Service, Donghae Dongin Hospital, Chuncheon Sacred Heart Hospital Affiliated with Hallym University, and Hongcheon Asan Hospital.
The integrated care linkage project for discharged patients is designed to establish a system in which hospitals can directly refer patients who have completed surgery or treatment to local governments for integrated care as they return home.
Through this system, eligible patients will be able to quickly receive the necessary care services from the moment of discharge, which is expected to contribute to reducing the rate of readmission and improving their quality of life.
Park Songlim, Director of Welfare and Health at Gangwon Special Self-Governing Province, stated, "This work agreement is significant in that local governments and medical institutions are cooperating to support residents’ return to a healthy daily life," adding, "We will continue to strengthen the integrated care system so that residents can maintain healthy and dignified lives in the communities where they were born and raised."
