Yangpyeong County Selected as "Integrated Care City"...Proactively Establishing a Comprehensive Medical, Long-term Care, and Housing Model

From Care to a Healthy Daily Life with "Yangpyeong Integrated Care"

Yangpyeong County in Gyeonggi Province (County Mayor Jeon Jinseon) announced on the 23rd that it was selected on the 20th for the "Integrated Care City" public contest project organized by Gyeonggi Province.

In advance of the enforcement next month of the Act on Integrated Support for Community Care, including Medical Care and Long-term Care, the county plans to use this public contest project to proactively establish a specialized integrated care model unique to Yangpyeong County.


The county has named the project "Yangpyeong, Spring Has Returned (Yangpyeong, DADOLBOM)" and will fully promote the "Yangpyeong-type Integrated Care Model," which organically links medical, long-term care, and care services within the community.


The main programs are: (1) "Neighborhood Visiting Care Primary Doctor," which provides necessary medical services at home; (2) "Daily Life Recovery Care Home," which supports the return to daily life after hospital discharge or leaving a facility; (3) "Daily Life Recovery Treatment Station," which offers intensive rehabilitation treatment; and (4) operation of an "AIP Coordinator," who supports stable living in the community.


Through this project, the county plans to enhance medical accessibility through the "Visiting Care Primary Doctor," provide intensive rehabilitation treatment via the "Daily Life Recovery Treatment Station," and prevent readmissions and re-entry into facilities through the "Daily Life Recovery Care Home." In addition, through the "AIP Coordinator," it will support care recipients so that, instead of entering facilities, they can maintain a stable home life in their existing place of residence by AIP (Aging In Place).


Residents can apply for the integrated support services through their eup or myeon community service centers, and the county will connect and provide customized services after selecting eligible recipients through counseling, assessments, and integrated support meetings.


County Mayor Jeon Jinseon of Yangpyeong said, "Being selected for this Integrated Care City public contest project will serve as an important opportunity for Yangpyeong County to lead a community-centered integrated care system," adding, "We will do our best to ensure that medical care, long-term care, and care services are organically connected so that every county resident can continue to live a healthy life where they have always lived."
