Gwangsan District in Gwangju Operates Counseling Room for Discharged Patients' Home Reintegration On the 27th, Gwangsan District conducted consultations with patients and their guardians at the "Visiting Discharged Patient Home Return Support Counseling Room" operated at Gwangju Veterans Hospital. Provided by Gwangsan District

On August 27, Gwangsan District opened its first counseling room at Gwangju Veterans Hospital. The counseling room for supporting discharged patients' return home is jointly operated by Gwangsan District and Gwangju Veterans Hospital. It is designed to allow patients to receive counseling and guidance on necessary care information at the hospital before discharge.

The purpose of the counseling room is to reduce care gaps that may occur immediately after discharge by providing customized design and connection to a variety of welfare and care services, such as health, meals, mobility, and housing, at patients' homes after discharge.

The counseling on this day was provided from 10:00 a.m. to 11:30 a.m. for Gwangju citizens who are hospitalized patients (or scheduled for discharge) and require care. It is evaluated that the accessibility and convenience of the system have improved, as patients and their guardians can receive counseling and application guidance from the period of hospitalization.

At the counseling room, the health status, care needs, and living environment of the participants were assessed, and explanations were provided about the discharged patient home return service, along with guidance on application and connection procedures.

Starting with the counseling room at Gwangju Veterans Hospital, Gwangsan District plans to expand the program to 22 partner hospitals in the region and to hold regular counseling sessions once a month.
