Agreement Signed with Five Local Hospitals
Supporting Community Reintegration of Discharged Patients

Gwangjin District in Seoul has joined forces with five hospitals in the area to support the stable reintegration of discharged patients into the local community. On June 2, the district signed a business agreement with Konkuk University Medical Center, Hyemin Hospital, Zenith Hospital, Dongbuchamsarang Nursing Hospital, and Pyeonanhan Nursing Hospital, pledging mutual cooperation and partnership.

This agreement is part of the 'Gwangjin District Integrated Care Support Pilot Project.' Its goal is to provide tailored services such as health care, medical support, housing, nursing, and care to patients who have been hospitalized for more than two weeks and are experiencing difficulties in daily life after discharge. By doing so, the project aims to prevent readmissions and help patients return to healthy daily routines.


Under the agreement, participating medical institutions will identify discharged patients who require care by assessing their physical functions and financial situations, and will refer these individuals to Gwangjin District. Based on these referrals, the district will establish individualized support plans and provide a range of integrated community care services, including home medical visits, health management, and daily living assistance.


Kim Kyungho, Mayor of Gwangjin District, stated, "This agreement is an important first step toward the full-scale implementation of the Gwangjin District Integrated Care Project, which will begin next year. We will do our utmost to ensure that discharged patients can continue to receive care and live healthily in the familiar environment they have always known."
