Plan to Establish 'Integrated Care Promotion System' to Provide a Foundation for Healthy Living in Vulnerable Groups' Residences
Gwanak-gu (Mayor Park Jun-hee) was honored as the only institution in Seoul to receive the Minister of Health and Welfare Award, selected as a meritorious organization for the 'Integrated Medical Care Support Promotion' led by the Ministry of Health and Welfare.
The district was recognized for its 'Gwanak-type Medical Care Specialized Project' based on regional characteristics. It laid the foundation for a customized integrated care system and effectively provided various 'health-welfare-care services' within the community through organic cooperation between the public and private sectors.
In particular, the district received high praise for projects supporting care blind spots, including ▲identifying borderline care targets ▲linking care SOS services for discharged patients from local hospitals ▲providing polypharmacy home counseling services for medical aid recipients and assigning dedicated care personnel.
Additionally, efforts have been made toward integrated care for the healthy aging of seniors through initiatives such as the ‘Gwanak-type Dementia Safe Village Creation - Dementia Noriter’ and the ‘Smart Senior Centers - Brain Youth Program’.
Health care projects for prevention, such as ‘Free Health Checkups for Single-Person Youth Households’ and the ‘Visiting Health Companion Project,’ have also been implemented.
The district plans to launch an ‘Integrated Care Promotion Team’ and establish an ‘Integrated Care System’ to support the healthy lives and aging of vulnerable care groups. The goal is to create a foundation that enables seniors, people with disabilities, and other vulnerable groups to live healthy lives in their own homes.
Mayor Park Jun-hee stated, “An integrated care system centered on the local community will be an important issue to strengthen customized welfare throughout the life cycle for residents’ happiness. We will continue to do our best to create an environment where optimal medical care services can be provided through mutual cooperation between the public and private sectors.”
