The Integrated Case Management Project is a program in which public and private sectors cooperate to provide tailored services for individuals with complex needs such as financial difficulties, health issues, and care, housing, or mental health crises. The project aims to resolve crises and enhance the self-reliance capabilities of those in need.

To achieve this, the city's Hope Welfare Support Team, along with customized welfare teams at the township, town, and neighborhood levels and private professional organizations, collectively manage every stage from case identification to service linkage and post-service monitoring.

This meeting was attended by the Hope Welfare Support Team, representatives from public and private case management agencies, and other related organizations. The participants comprehensively evaluated the 2025 operations of integrated case management and discussed the 2026 operational plan. The agenda included a review of major achievements in 2025, measures to enhance the operation of integrated case conferences in 2026, strategies to strengthen public-private cooperation, and plans to expand the linkage of local resources.

A Yangsan City official stated, "Integrated case management goes beyond simply connecting services. It is a crucial initiative in which multiple organizations share responsibility to bring about real changes in the lives of those in need. Based on the operational achievements in 2025, we will proactively identify households in crisis and build a robust social safety net through even more systematic public-private cooperation in 2026."
