Full-scale Shift to Community-based Care System
Kimcheon City has officially launched its "community-based integrated care" system, which closely connects medical care and social support, marking its first step in shifting from hospital-centered care to a model focused on residents' daily lives.

On January 19, Kimcheon City (Mayor Bae Nakho) announced that it had signed a business agreement with Kimcheon Medical Center to implement the "Long-term Care Home Medical Center Pilot Project" and the "Discharged Patient Community Coordination Project."
This agreement is notable as it marks the first practical phase in which Kimcheon City is putting its integrated care policy into action at the field level, following the establishment of a dedicated Integrated Care Team through a 2026 organizational restructuring.
The integrated care model is a new approach that enables citizens in need of support, such as the elderly and people with disabilities, to receive comprehensive medical, long-term care, social support, and daily living assistance services in their own homes, without having to enter a nursing facility or be hospitalized. The core goal is to reduce service gaps caused by separate medical and welfare systems and to help people maintain a healthy daily life in their own communities.
Under the agreement, the Long-term Care Home Medical Center will provide home visits by a multidisciplinary team consisting of doctors, nurses, and social workers, delivering home medical care and nursing services. It will also integrate local care resources to offer comprehensive support, including medical, long-term care, and daily living assistance services.
In particular, the Discharged Patient Community Coordination Project is designed to minimize the "care gap" that can occur immediately after hospital discharge. It will quickly connect discharged patients to the community integrated care system, ensuring they continue to receive necessary medical and daily living support.
Starting with this agreement, Kimcheon City plans to form an integrated support council, expand its cooperative network with local medical and welfare institutions, and gradually broaden its integrated care projects to encompass health care, daily living, and housing support.
Bae Nakho, Mayor of Kimcheon City, stated, "This agreement represents the first step for Kimcheon City, which has laid the foundation for integrated care, to join hands with medical institutions and move into implementation. We will continue to work with the local community to build a robust integrated care system, so that citizens can live safely and confidently in the places they call home."
