Full-scale Launch of 'Back Home' Project to Support Discharged Patients' Return Home

Seo-gu District in Gwangju Metropolitan City is taking steps to support home living for elderly residents with mobility difficulties.
On January 15, Seo-gu District held a 'Long-term Care Home Medical Center Pilot Project Agreement Ceremony' with Taeyoung 21 Internal Medicine Clinic at the Seo-gu Office Eum Hall to strengthen the home visit medical support system.
This agreement was promoted to expand the foundation for home visit medical services, enabling medically vulnerable groups to receive stable treatment and care in their own residences.
The two organizations will cooperate on all aspects of home medical care, including home visits, health management, and chronic disease management.
Seo-gu District was the first in the nation to launch a home medical center pilot project in July 2022. Currently, two medical institutions-Cheomdan Family Union Clinic and Senior Integrated Clinic-along with the Seo-gu Public Health Center, are participating in its operation.
For long-term care recipients with limited mobility, doctors and nurses visit homes to provide medical services, while social workers connect integrated care resources, offering a comprehensive 'medical-care-welfare service.'
Last year, 795 people received a total of approximately 16,000 home visit medical services.
The district also assists discharged patients in returning home. Through the 'Back Home' project, Seo-gu District plans to minimize care gaps and prevent readmissions by providing short-term intensive services such as accompaniment and transportation support at discharge, as well as home visit medical care, housekeeping, meal, transportation, and housing support.
Seo-gu District Mayor Kim Ikang stated, "We will strengthen the foundation for home visit medical care through cooperation with home medical institutions and, in connection with the support project for discharged patients, ensure that residents can safely return to their daily lives."
Meanwhile, Seo-gu District is leading integrated care by establishing a 24-hour care system and a public medical care model, and has been recognized as an excellent local government for six consecutive years in the Ministry of Health and Welfare evaluations.
