On the 15th, Dongdaemun District announced that it had signed a business agreement for the 'Discharged Patient Community Connection Project' and will strengthen its integrated support system to quickly connect residents in need of care after discharge from the hospital to community services.
The agreement includes six general hospitals-National Medical Center, Seoul Medical Center, Seoul Metropolitan Dongbu Hospital, Kyung Hee University Hospital, Sahmyook Seoul Hospital, and Seoul Sacred Heart Hospital-as well as two long-term care hospitals, Haetsal Nursing Hospital and Hyolim Nursing Hospital.
The district explained that this agreement has expanded the pathway for integrated medical and care support projects between hospitals and the district. The aim is to reduce cases where patients in need of care immediately after discharge are excluded from support due to lack of information or unfamiliarity with procedures and to establish a connection-based system that enables step-by-step support from recovery management to community reintegration.
Lee Philhyoung, Mayor of Dongdaemun District, stated, "With this agreement as a starting point, we expect hospitals and the community to be connected more efficiently, allowing Dongdaemun District's integrated care services to advance to the next level. We will be a strong pillar so that those eligible for integrated care can continue to live healthy and happy lives in their own homes."
