Formation of the "One-Stop Health and Medical Care Team"
Integrated Provision of Medical, Care, and Welfare Services
Siheung City in Gyeonggi Province announced on August 22 that starting next month, it will launch the "One-Stop Health and Medical Care Pilot Project," which provides integrated home care services for vulnerable groups with mobility difficulties or health issues.
The project will operate by dispatching the "One-Stop Health and Medical Care Team," which is composed of doctors, dentists, nurses, social workers, physical therapists, dental hygienists, and nutritionists based at the public health center, to visit homes and provide integrated medical, care, and welfare services.
Through this initiative, the team will comprehensively manage the entire process, including patient classification and health assessment, development of customized care plans, regular home visits and nursing, provision of rehabilitation, nutrition, and oral care services, as well as linkage to welfare services and follow-up management.
If specialized medical treatment is required, the city plans to address gaps in community healthcare by continuously managing patients in cooperation with the Visiting Care Medical Center (Sincheon Union Hospital) and the Long-Term Care Home Medical Center (Hope Clinic).
Lee Hyeongjeong, Director of Siheung City Public Health Center, stated, "This pilot project is the first step toward comprehensively caring for the health and daily lives of each citizen, going beyond just treatment," adding, "Based on the results of this project, we will establish a foundation that enables all citizens to access medical care and support without discrimination."
