'Songpa Sarang Health Doctor' Program Where Doctors Visit Patients Directly to Start from April


From doctors to nurses and exercise therapists, a medical professional team visits residents directly.


Songpa-gu (Mayor Seo Gang-seok) health centers announced the introduction of the 'Songpa Sarang Health Primary Care Physician' program, where doctors visit and provide medical care directly to vulnerable groups such as elderly residents with mobility difficulties.


The 'Songpa Sarang Health Primary Care Physician' program forms a team of eight professionals working at the health centers, including doctors, nurses, exercise therapists, nutritionists, physical therapists, occupational therapists, clinical pathologists, and dental hygienists, who visit each household to provide a one-stop medical service tailored to the health conditions of the residents.


The district explained that due to the super-aging population and the increase in single-person households, the number of elderly and disabled individuals unable to visit hospitals has risen, prompting the implementation of home visit medical services to establish a thorough medical welfare system.


Until now, nurses conducted basic health checks through the 'visiting management service,' but with the new 'Songpa Sarang Health Primary Care Physician' program, a team of experts led by doctors will visit homes to provide professional and integrated diagnosis and treatment simultaneously.


The support targets about 100 residents, including high-risk groups identified and referred by visiting health team nurses, and those aged 65 or older with chronic diseases such as hypertension, diabetes, and dyslipidemia.


Home visits involve ▲ a primary preliminary survey to assess the patient's condition, and ▲ a secondary visit by the medical team including doctors, nurses, and exercise therapists to provide systematic medical services ranging from diagnosis to nutritional therapy, exercise therapy, and physical therapy.


After the secondary treatment, an optimized treatment team tailored to each patient's characteristics will be formed, with specialists from each field creating personalized health management plans, continuously monitoring progress, discussing issues, and providing team feedback to meticulously manage residents' health.


Additionally, the district is working to eliminate medical blind spots by offering health promotion services such as 'visiting health consultation rooms,' 'mobile smoking cessation clinics,' 'early dementia screening,' and 'home rehabilitation services for the disabled.'


Seo Gang-seok, Mayor of Songpa-gu, stated, "This 'Songpa Sarang Health Primary Care Physician' program will be a great support for elderly and disabled residents with mobility difficulties," adding, "We will continue to provide thorough health and medical services so that more residents can benefit and everyone in the district can enjoy a healthy life."
