This pilot project is operated by having a home medical care team?composed of doctors, nurses, social workers, and physical therapists?visit the homes of eligible recipients of long-term care home-based benefits (grades 1 to 5) who require house calls, providing medical services and linking them with community care services.

According to the agreement, Pohang City is responsible for identifying and connecting eligible recipients and providing care services, while Naejibeseo Clinic is in charge of house calls, home nursing, home rehabilitation, and linking customized care services.

Kang Hyunju, Director of Welfare Policy, said, "As Pohang enters a super-aged society, we will do our best to ensure that seniors can continue to live healthy lives by cooperating with Naejibeseo Clinic to minimize any gaps in medical care."

Pohang City plans to hold ongoing business coordination meetings to establish a community-integrated care system and to provide support so that seniors can maintain stable lives without any gaps in medical care through close cooperation with relevant organizations.
