The Gokseong-gun Dementia Relief Center in Jeonnam announced on the 11th that it is providing customized case management services tailored to the individual needs of dementia patients.
Customized case management aims to eliminate care blind spots for dementia patients and improve their quality of life by systematically addressing issues related to physical, mental, and social needs.
Eligible recipients are selected from among dementia patients registered at the Dementia Relief Center who are in care blind spots, upon request from the patient or guardian, or upon referral from other organizations. Selection is based on an assessment of complexity, severity of issues, stability of condition, and urgency of intervention.
The county is currently monitoring 350 out of 1,227 registered dementia patients as recipients of customized case management. These individuals receive hygiene supplies such as incontinence pants, fall prevention items such as non-slip socks and canes, and are provided with regular counseling and home visits.
The center holds a Dementia Case Management Committee and case meetings once a month to discuss the direction of service provision through deliberations on selection, termination, and extension of recipients. The committee consists of five members, including a collaborating physician from the Dementia Relief Center (a psychiatrist from Gokseong Sarang Hospital) as chairperson, the head of the mental dementia team, and other staff.
A representative from the Gokseong Dementia Relief Center stated, "The center will continue to do its best to provide services tailored to each individual recipient."
