Checklist Format, Resolving Public Inconvenience

On the 17th, the National Police Agency's National Investigation Headquarters announced that it has prepared a simplified complaint form for major crimes such as fraud, defamation·insult, and assault.

The existing complaint forms had no fixed format, allowing complainants to freely write the facts of the crime and reasons for the complaint, which caused difficulties for ordinary people unfamiliar with legal terminology. Additionally, since it was unclear what information was necessary, many cases involved omitting essential elements or writing unnecessarily lengthy content.


The simplified complaint form uses easy-to-understand terms so that anyone can comprehend it, and it is structured as a checklist-style questionnaire covering the necessary items for filing a complaint. Even if the complainant does not know the defendant’s address or contact information, they are instructed to provide any known information such as identification (ID), nickname, or bank account number. The form also separates sections to briefly record the details of the damage by type, including date, place, and content.

An investigative officer on the front line commented, “Currently, many complaints received lack specific details such as date and place because complainants write without a fixed format. Some complaints are so vague that it is difficult to grasp the facts of the crime, which has caused many challenges. Having a standardized complaint form like this will likely aid investigations.”


The simplified complaint form can be used at civil affairs offices and investigative civil consultation centers at police stations nationwide, and it is available on the National Police Agency’s website.


Woo Jong-su, head of the National Investigation Headquarters, stated, “The simplified complaint form will help resolve public inconvenience while also improving police officers’ work efficiency. We will continue to do our best for people-centered investigative activities.”
