Hwasun County to Launch Full-Scale 'ICT Convergence Home Visit Health Management Project'

Providing Integrated In-Person and Digital Personalized Health Management Services

Hwasun County in South Jeolla Province announced on January 23 that it will further advance its ongoing home visit health management program by launching the full-scale 'ICT Convergence Home Visit Health Management Project' starting in 2026, which incorporates information and communication technology (ICT).

The home visit health management program involves healthcare professionals, such as nurses, visiting the homes of vulnerable individuals to check their health status, including blood pressure and blood sugar levels, and to provide personalized health consultations and education.

In particular, the program also supports overall health and safety in daily life, such as education on chronic disease management and safety checks for heat waves and cold spells. Through regular home visits, it serves as a reliable guardian of seniors' health.

Beginning in 2026, the county plans to implement an AI and IoT-based senior health management program in conjunction with the existing home visit health management project.

This new initiative will lend ICT-based devices-such as wrist-worn activity trackers and Bluetooth-enabled blood pressure and blood glucose monitors-to individuals who require prevention and management of frailty and chronic diseases. Health data transmitted via a dedicated app (Oneul Health) will be continuously monitored by public health center professionals, who will promptly provide telephone or in-person consultations and medical referrals if any abnormalities are detected.

By moving beyond the traditional model centered on regular visits to a system of continuous monitoring, the county aims to minimize gaps in care and encourage individuals to participate in managing their own health, thereby strengthening their self-management capabilities.

Park Mira, Director of the Public Health Center, stated, "The ICT Convergence Home Visit Health Management Project is a proactive health management model in response to a super-aged society. Through the seamless combination of the warm, in-person 'Home Visit Health Management Program' and the smart technology-driven 'AI and IoT-based Senior Health Management Program,' we will do our utmost to safeguard the health of our seniors."
