The target of the home health care project includes citizens who need to improve health behaviors such as smoking and frequent drinking or have health problems such as chronic diseases like hypertension and diabetes. Priority is given to socioeconomically vulnerable groups such as basic livelihood security recipients, lower-income groups, and elderly living alone, as well as those aged 65 or older who have been assessed as outside the elderly long-term care grade.

Five specialized nurses are deployed for the home health care project, providing 1:1 customized home health care services by classifying about 3,500 health-vulnerable individuals into intensive management groups, regular management groups, and self-capacity support groups. During visits, basic tests such as blood pressure and blood sugar are conducted, customized health education and counseling are provided for each subject, and health management supplies including nutritional supplements and patches are distributed.
