A Healthy Old Age in the Community Where They Live
Seongju County in North Gyeongsang Province is accelerating the promotion of an integrated medical and care support project so that senior citizens can enjoy a healthy old age in the communities where they have been living, in line with the Act on Supporting Community-Based Integrated Care such as Medical and Long-Term Care, which will be fully implemented from March 27.
Over the past year, the county has operated a pilot project, actively identifying blind spots in care, providing customized services by linking local resources, and accumulating a wide range of experience. During the pilot period, a total of 21 cases were identified, and a total of 42 services were coordinated across five areas, including home medical care, home health management, and care services, tailored to the needs of the beneficiaries.

Based on these achievements, the county plans to concentrate its administrative capacity on establishing a service system to ensure the successful settlement of the main project. In particular, it will hold meetings with eup and myeon officials before the project is implemented to listen to voices from the field, and will carry out extensive promotional activities so that county residents can understand the details of the project.
In addition, its key tasks include: recruiting service providers that can offer a variety of daily living support services; signing business agreements with local hospitals to facilitate the smooth return of discharged patients to the community; improving the residential environment to ensure safe housing; and operating a home medical center for seniors with limited mobility.
In particular, the Discharged Patient Linkage Project, which connects hospitals with the local community, is expected to play a key role in preventing cases of patients being readmitted to hospital due to gaps in care after discharge.
An official from Seongju County said, "Going forward, we will further strengthen cooperation between local medical institutions and care providers, and build an integrated medical and care support system that county residents can truly feel."
